Hello, my name is Ms. Norton.  As Lead Medical person at Pathways New Connections, I am here to focus on person-centered education relating to Individuals diagnosis and treatment plans; educate Individuals on self-administering medications; administer medications as prescribed, and document when given with time and initial; monitor Individuals health, well-being and records signs of changes with follow-up as needed; and advocate for the health and well-being of individuals and provide emotional support to them and caregivers as needed. As your career service provider, we are aware that with the COVID-19 infection, individuals with immunocompromised states or with medical problems like high blood pressure, heart problems, diabetes, are more likely to develop serious illness.  While social distancing and other measures to prevent infection are of the upmost importance, it is equally important to remain healthy and keep your pre-medical conditions well controlled.  You can do this by continuing regular follow ups with your primary care doctor.  Pathway New Connections is here to support you.

Dear New Connection family/caregivers and Providers,

As a valued member of our Pathway community, we appreciate the trust you place in us and want to share how we are addressing the coronavirus (COVID-19) situation.

We will continue to closely monitor events in our local community and continuously update our policies and protocols as a result of the news.

Please know that our center is following all recommended guidance from public health authorities, including best practices for servicing our individuals and staff team.  We feel confident in our ability to continue servicing our individuals in a safe and nurturing environment.

Our highest priority is to keep all our individuals and staff as safe as possible.

Therefore, at the recommendations of CDC, we will be taking individuals and staff temperature and asking these screening questions prior to entering our center.  Please be prepared to answer the following questions.

  • Have you traveled to a COVID-19 affected geographic area within 14 days?
  • Have you had close contact with a person who traveled to high risk sites or has COVID-19?
  • Do you have fever and respiratory symptoms such as cough or shortness of breath?

If you answered yes to any of the above questions, you will be asked to return to your vehicle for further evaluation from your primary physician.

These recommendations are put in place with our efforts to help determine whether additional treatment is required.
